Where the claim is for money, the claim form must also contain the statement of value referred to in rule 16.3.
2.2 The claim form must include an address at which the claimant resides or carries on business.
3.2 Where the claimant does not include the particulars of claim in the claim form, particulars of claim may be served separately: (1) either at the same time as the claim form, or (2) within 14 days after service of the claim form.
